I called back number 0420 973 960 spoke to a woman who said company name is Choosi Insurance.

rang me heaps never leaves messages when called back no answer further investigation found out its choosy insurance.

Yep it could very well be choosi. Rang a few times to follow up some info didn't leave message and was unable to contact them via this number .... so I blocked it... they seem to be very focused on selling their goods.. probably too much

It was in response to a web form that I filled out... and they proceeded to bombard me with calls, so I blocked them.
